SMS Web Form Opt-In Proof
This page documents the web-form opt-in flow XLEO uses to collect SMS consent from end users. It is intended to serve as public proof of the disclosure language, checkbox flow, and recordkeeping used for text message enrollment.
How End Users Opt In
End users opt in by completing a web form hosted by or on behalf of XLEO. The form requires the user to provide their mobile number and actively check a consent box before SMS enrollment is submitted.
- The consent checkbox is optional and is not pre-checked.
- The submit action is not treated as SMS consent unless the checkbox is selected.
- The form is used for operational, support, approval, and account-related messaging only.

Consent Records Maintained
For each SMS opt-in collected through the web form, XLEO retains records such as:
- Name and mobile number provided by the end user.
- Date and time of the form submission.
- The page or workflow where consent was collected.
- A record that the consent checkbox was selected at submission time.
